Post-Treatment Phase
After the initial treatment has been completed, the client enters the post-treatment phase when the Nurse Case Manager provides:
- A detailed summary of the treatment course
- Suggestions on how to promote recovery and reduce cancer reoccurrence
- Advice on lifestyle changes required to reduce risk of other chronic diseases
- Tools to enable the plans to work effectively
- A guide to future health care
