Cancer Assistance and Elder Care
Carepath’s Cancer Assistance program was started in 2007 by two leading oncologists who were concerned their patients were not getting enough help to manage their illnesses. The cancer care system was highly fragmented, leaving patients with many questions and concerns but few clear paths to address them, especially when they were not feeling well. A cancer diagnosis can be frightening and overwhelming, and treatments often extend over many months.
They knew that having an Oncology Nurse Navigator who could assist patients in finding their way through the system, provide support, answer questions, and serve as compassionate listeners when needed.
With Bayshore as our parent company, we recognized it made sense to expand into services for seniors. In 2016, we added our virtual Elder Care navigation program to assist seniors, and their families navigate senior services.
Studies have shown that patients with navigators tend to do better and achieve improved outcomes. Although some provinces established cancer navigator positions, gaps remain in the system.
Chronic Disease and Mental Health Programs
As the rates of chronic disease in Canada continued to escalate, we launched our Chronic Disease program in 2017, providing navigation for all acute and chronic illnesses.
Our Mental Health program was added in 2020. More than a navigation program, we have a team offering counselling and psychotherapy also – to individuals (adults, adolescents, and older children), as well as couple and family therapy.At our heart is a commitment to helping people find their way while offering answers, advice, and supportive, compassionate listening.
