I have had depression my whole life and I am now in my 60s. I knew that my condition was chronic but still hoped to feel a bit better. My mental health clinician worked with me to design a plan that added to what I had already done in the past. We used CBT and ACT mostly, which included lots of breathing and meditation exercises. She introduced me to the Carepath mobile app, and the “Mind Zone” program. It’s an 8-week program for anxiety/depression that’s easy to use and gave me tools at my fingertips. Everything in the program was reinforced in my appointments and vice versa so it was an easy to use reminder of things we talked about and exercises I needed to practice. In the end, we worked a lot with a fear ladder as there were a lot of places and situations I was inclined to avoid and that caused me anxiety. When we got to the end of therapy, I was not finished but I knew what to do to keep improving on my own and afterward found my confidence continued to build. Therapy is “the gift that keeps on giving” as long as you persist!

As a 36-year-old married father of two, I didn’t know that low energy, trouble sleeping and fighting with my spouse was related to anxiety and depression until I contacted Carepath for help. My mental health clinician created a treatment plan with me and helped me to identify some goals. My plan included medication and Cognitive Behavioural Therapy (CBT). I was able to access virtual care for my weekly CBT counselling sessions, and my mental health clinician communicated with my family doctor throughout, something I wanted. My spouse and I were also referred to couples therapy and our relationship is improving. Plus, I was able to return to work after 8 weeks, reassured that I could reconnect with my therapist, if needed.

I contacted Carepath when my 70-year-old mother with early onset dementia needed to be moved into assisted living. I wanted her to be close to family, but that meant moving her from Halifax to Toronto, and I had no idea where to start on making the arrangements. From ensuring my mother had a PSW in Halifax to facilitating the OHIP application and arranging the transport to Toronto, my Nurse Case Manager assisted me with all the details by connecting me with all the right people and places. I then got help finding a long-term care home and was able to move my mother into a place she felt supported by the staff and her family. I thought I would have to take a leave from work to take care of my mother, but my Nurse Case Manager helped me feel empowered to make informed health care decisions, so I could continue working, knowing I was doing the best for my mother’s quality of life.

We wanted to let you know how much we have appreciated Brianna’s support!
Brianna has really helped clarify our need for areas to work on with Drs. Appointments.
She helped us organize with ideas and options for things available to assist us with our care!
She is professional in every area; she retained our areas of concern listened then had many options for alternatives.
Since we both have needed extra care and appointments this fall and early winter we need to spend time with her to go over those results and where we might access some further support.
Dr appointments right now are taking many weeks, some months and one over a year so waiting for them has been on trying for both of us.
For example, my husband had not seen his GP for over six months and needed medication renewals he wasn’t allowed to see any other Dr in the clinic. That appointment just happened.
Brianna over these months has been a gentle caring listening and exploring nurse who had very knowledgeable information.
But we need and appointment now with her to go over what has occurred.

I contacted Carepath as I was looking for support and resources to help support my aging parents. I was contacted by Suzanne one of the Nurse Case Managers of the Eldercare Program who provided me with excellent online resources. What I was most impressed with; however, was the conversation I had with her over the phone. She gave me excellent advice as to how to support both my parents following the results of a geriatric assessment that determined my father had Alzheimer’s. My aging mother is his caregiver. Suzanne helped me understand and use appropriate language to ensure my mom’s needs were also supported. Suzanne helped greatly with navigating the overwhelming feelings of this change in my parents and provided specific strategies to also help my parents accept this. Since the time I reached out to Carepath, we have accessed more support from the resources Suzanne shared. I am so glad I reached out to Carepath and so appreciative of the support provided by Suzanne.

I was very concerned about the onset of new back and neck pain and how it was impacting by ability to function. When I was diagnosed with possible myelomalacia – softening of the spine – my Nurse Case Manager really helped me understand my diagnosis and treatment plan. With ongoing phone support, we also discussed other potential causes of the pain, alternative treatments for my pain like physio and massage, and what my MRI results meant. My Nurse Case Manager also helped me prepare for appointments by ensuring I had the right questions to ask and access to local community services during the pandemic. Carepath’s larger network even helped me secure local specialist appointments faster. Thanks to my Nurse Case Manager, my spouse and I felt supported and in control and I was able to remain actively at work throughout diagnosis and treatment.
