Client Rights

Carepath is committed to providing highly skilled clinicians with the appropriate health care skills and expertise to meet your needs. As a client or patient of Carepath you have several specific rights and responsibilities. These rights are consistent with provincial and federal health care legislation. It is important that you fully understand these rights and responsibilities, as these will form the basis of your care. Should any of these be unclear, please email carepathintake@bayshore.ca and we will be able to answer any questions.

All Carepath clients, patients, and/or their representatives have the right:

  1. To receive care, free from discrimination as per the Human Rights Code or the Canadian Charter of Rights and Freedoms.
  2. To be fully informed of their rights and responsibilities before the initiation of care or service.
  3. A client or patient who is First Nations, Métis or Inuk has the right to receive care or service in a culturally safe manner.
  4. To be dealt with in a manner that respects their dignity and privacy and promotes their autonomy and participation in decision-making.
  5. To be dealt with in a manner that recognizes their individuality and that is sensitive to and responds to their needs and preferences, including preferences based on ethnic, spiritual, linguistic, familial and cultural factors.
  6. To receive the appropriate care from a provider who respects and considers individual needs and promotes a person- and family centered approach to care and service.
  7. To be recognized as the leader of your care and to acknowledge the partnership and role of your family to the degree you desire.
  8. To receive care in consideration of your safety and well-being from clinicians who are fully qualified and experienced to provide the care required.
  9. To receive continuity of care.
  10. To receive a timely response to a request for service.
  11. To privacy and confidentiality of personal information. Carepath collects, uses, discloses, retains, and destroys personal information as appropriate while providing care and in keeping with sound information management practices and privacy legislation.
    Please email Carepath’s Privacy Lead, privacyofficer@bayshore.ca, with any privacy concerns. Carepath’s Privacy Lead will escalate any privacy concerns to the Privacy Officer of Bayshore HealthCare Ltd if further clarification is required.
  12. To obtain access to or request correction to their personal information. Requests are to be submitted to Laura Beatty, Managing Director in writing at lbeatty@bayshore.ca. Assistance will be provided if needed. At the time of a request, additional personal information may be required to validate the identity and authority of the individual making the access request. This additional information will only be used for this purpose. Carepath endeavors to respond to all requests for access to personal information within 30 days. Access to information may be subject to limitations or refusal. You will be notified of the reasons for refusal, including recourses available to you if this is the case.
  13. To receive all necessary pertinent information to provide informed consent before the initiation care.
  14. To refuse care, as defined in provincial and federal legislation, and to be free of the potential consequences of any such refusal.
  15. To raise concerns or recommend changes related to the care that you receive, and with policies and decisions that affect your interests, without fear of interference, coercion, discrimination, or reprisal.
  16. To make a complaint, comment, question, inquiry, or compliment about Carepath practices. To exercise this right contact Laura Beatty, Managing Director at lbeatty@bayshore.ca
  17. To receive accurate, honest, and clear information on any aspect of Carepath’s services.
  18. To ensure Carepath clinicians are treated in a respectful manner, free from violence, harassment, bullying and discrimination, and acknowledge that Carepath has the right to suspend or terminate services if not compliant with this responsibility.
